Enrollment Requirements & Agreements

To successfully enroll and complete our CNA/STNA, you must understand and agree to the following statements.

• I am of legal working age in the State of Ohio (16 years old or older).

• I understand I will need to present a valid US government photo identification (drivers license, permit, CCW, state ID, passport, etc.).

• I understand I will need to present an original social security card (no pictures or copies) for clinicals

• I understand I will need an 1-Step Tuberculosis screening for clinicals.

• I understand I will need a valid physical (completed within current year)

• I understand I may need to show proof of a COVID-19 vaccine or a signed waiver.

• I understand I am required to wear TEAL, GREY or BLACK scrubs to class/labs and clinicals. 

• I understand I will need to sign a scrub refundable $25 rental agreement, if unable to provide my own set of teal scrubs.

• I understand this is a hybrid (online & in-person) course and will need a reliable computer/device (laptops, desktops, tablets, etc.) with internet and printer access to complete assignments and/or exams.

• I understand that class, labs, and clinicals are in-person and will adhere to the chosen schedule.

• I understand that class, labs, and clinicals are 100% mandatory, missing these will result in an incompletion of the training.

• I understand that if a class, lab or clinical is missed, I will be required to pay a $50 makeup fee for each day that is missed.
